Registration Requirements

Please have the following documents in hand in order to efficiently register your child at GMS.

1. Two Proof's of residency (Power bill, Gas bill, Driver's License, Voter ID,
   Tax receipts, Mortgage/Lease documentation)
2. Withdrawal form - only if withdrawing during the school year
3. Last report card
4. Gifted/IEP documentation (if applicable)
5. Birth certificate
6. Social Security card
7. Immunization record is needed at time of enrollment.  Please submit on Georgia form #3231 and ear, eye, and dental on Georgia form #3300.
8. Custody / guardianship paper (if applicable)
9. Standardized Test Data - helpful in appropriate placement
